Financial Assistance Program

Billing Information and Collection Policy

Anticipating surgery can be stressful, and part of the anxiety can arise out of not fully understanding out-of-pocket expenses associated with a proposed surgery. At Florida Endoscopy and Surgery, we believe it is important to make patients aware of their potential financial expense prior to their procedure.

One of our insurance verification specialists will contact you if there are any anticipated out-of-pocket expenses associated with your planned procedure. Please realize charges for your procedure at Florida Endoscopy and Surgery are separate from your surgeon’s charge as well as any anesthesiologist and/or pathologist charges which may be incurred. Florida Endoscopy and Surgery collects all co-payments or deductible amounts on the day of your procedure. If your procedure is not covered by insurance, full payment is required at the time of service unless other arrangements are made in advance. Florida Endoscopy and Surgery does not offer charity care or financial hardship discounts. If you are unable to cover the cost of your surgery, please contact Florida Endoscopy and Surgery at 352-576-5065 prior to your date of surgery so further arrangements can be made.

Patients, or their legal guardians, are responsible for payment of facility services. As a courtesy, Florida Endoscopy and Surgery will bill your primary and secondary insurance carriers for you. Please bring your insurance cards with you on the day of surgery.  Uninsured and cash-paying patients will be required to pay for services prior to or on the date of your procedure.

Florida Endoscopy and Surgery accepts cash, checks, major credit cards, flexible spending accounts (FSA), health savings accounts (HSA), and care credit as financing options. Your Surgery Center Fee Includes:

  • Medical and surgical care
  • Cost of routine equipment and supplies
  • Cost of drugs administered (including anesthesia drugs)
  • Operating room time
  • Recovery room services

Anesthesia, pathology, surgeon, and physician consultation fees are not included in the Center’s fee and may or may not participate with the same health insurers or health maintenance organizations as the surgery center.

Prior to your procedure, you may request from the surgery center and other health care providers a more personalized estimate of charges and other information. The estimate is based on your current insurance benefits. Patients should contact each health care practitioner who will provide services in this surgery center to determine the health insurers and health maintenance organizations (HMOs) with which the health care practitioner participates.

Following your procedure, upon request, you may request an itemized bill of charges.

During the care you received at our facility you may have received services from physicians or other medical providers, such as Anesthesiologists, CRNA's, laboratories, etc.  You will receive separate bills from these physicians or medical providers. Should you have any questions concerning their bill, please contact them directly.
CARE BUNDLES
To view information on payments made to the facility for service bundles and procedures, you can visit the Agency for Healthcare Administration’s pricing website at price.healthfinder.fl.gov. Service bundle information is a non-personalized estimate of costs that may be incurred for the anticipated services; the actual costs charged will be based on the services actually provided to each patient. Patients and prospective patients may request from this facility and other health care providers a more personalized estimate of charges and other information. Patients and prospective patients should contact each health care practitioner who will provide services in the ASC to determine the health insurers and health maintenance organizations with which the health care practitioner participates as a network provider or preferred provider.
